(30 hours per week,1 year Fixed Term, NJC point 25, £36,363 FTE)
Bolton Carers Support is seeking a proactive and compassionate Health Partnership Co-ordinator to strengthen links between health services and support for unpaid carers across Bolton.
Working closely with GP practices, hospitals, community health services and the voluntary sector, you will raise awareness of unpaid carers, improve early identification, develop effective referral pathways, and ensure carers can access the support they need.
You will build strong partnerships, deliver training and outreach, engage directly with carers, and contribute to improving health and wellbeing while helping to prevent crisis.
The successful candidate will be an excellent communicator with experience of partnership working, community engagement and person-centred support. You will be organised, confident working across multiple settings, and committed to delivering inclusive, trauma-informed services that make a real difference to carers’ lives.
